You are invited to participate in a research project entitled, “West Virginia Provider Survey on Pain Management and Medication-Assisted Treatment,” which seeks to explore the experience of providers in West Virginia on the treatment of chronic pain, acute pain, and medication-assisted treatment of substance use disorder. This study is being conducted by Todd Davies, PhD of Marshall Health, and co-investigator Norman Montalto, DO of West Virginia Family Health, in conjunction with the West Virginia Clinical and Translational Science Institute and the Center for Rural and Community Health at the West Virginia School of Osteopathic Medicine.

This survey should take 20-30 minutes to complete. Your participation and your responses will be anonymous, so please do not put your name anywhere on the form. No information you provide can be linked to you or identify you as a participant. There are no known risks for participating in this survey. Participation is completely voluntary, and you may choose to not participate or to withdraw your participation at any time without penalty. If you choose to not participate in this survey, you may delete both the email and survey link. You may choose to not answer any question by simply leaving it blank.
